To help protect you and your family from the unforeseen financial hardship of a serious accident that causes death or dismemberment, UC offers retirees and their spouse or same-sex domestic partner Accidental Death and Dismemberment (AD&D) coverage through the UC-sponsored Voluntary Group Accident Insurance Program.
Plan Benefits
The plan provides benefits if the accident results in:
- accidental death or dismemberment or loss of sight
- paraplegia, quadraplegia, or hemiplegia
- coma lasting more than 30 days

You may enroll at any time. Use the form available from American Home Assurance.

To File a Claim
You or your beneficiaries should send a written notice of claim to American Home Assurance within 20 days of your loss. Call 1-800-551-0824 to request a claim form.
Naming/Changing Beneficiaries
To name or change your designated beneficiary, call the insurance company to receive a change of beneficiary form in the mail. Once the insurance company accepts a new form, all previous designations are revoked.
