VISION SERVICES PLAN (VSP)

The vision plan is open for enrollment.  Retirees who are eligible to enroll in VSP will receive an enrollment kit in the mail from VSP.  You must enroll directly with VSP by:

• Going online
• Calling VSP at 800-400-4569
• Completing a paper enrollment form and mailing it to VSP no later than November 24, 2009.

Note:  Retirees already enrolled in VSP will not receive an enrollment kit in the mail.  If you wish to make changes to your enrollment, such as adding a family member, you may do so online or by calling VSP (800-400-4569).

Retirees pay the full cost of vision insurance directly to VSP.  Rates for 2010 are:

Plan changes for 2010:

• The discount on additional pairs of prescription and non-prescription glasses, including sunglasses, when purchased the same day as the member’s eye exam from the same VSP doctor who provided the exam is increasing from 20% to 30%.  Members will still receive 20% discount for unlimited additional pairs of glasses if purchased through any VSP doctor within 12 months of the last covered eye exam.
• A Diabetic Eye Care Program, which provides medical eye care services specifically targeted to member with Type 1 diabetes, is being added.
