Plan changes for 2011:

• Generic prescription drug copay reduced from $10 to $5 (retail) and $20 to $10 (mail order).
• The copay for Urgent Care at any network provider is $15.
• There is no longer a lifetime maximum for in-patient mental health treatment days.
• The Medicare prescription drug calendar year out-of-pocket maximum increases from $4,350 to $4,550 per member.
