CIGNA CHOICE FUND HRA + PPO

The CIGNA Choice Fund is being discontinued. The Anthem Lumenos PPO with HRA will replace the CIGNA plan. 

Current CIGNA members will be transferred automatically to the new Anthem Lumenos plan unless they choose another plan during Open Enrollment.  If you choose Anthem Lumenos:

• Any remaining balance in your CIGNA HRA will roll over automatically to your Anthem Lumenos HRA in April 2011
• If you have CIGNA claims from 2010 for which you want to use your HRA, be sure to file them with CIGNA by March 31, 2011
• You may be able transition open prescription refills to your new plan without obtaining a new prescription. Check with Anthem.
• If you or a covered family member has an acute medical condition, are expecting surgery early in 2011, or are pregnant, call the Anthem customer service to review your conditions and any preauthorization requirements.
