BROWN & TOLAND MEDICAL GROUP ENDS AFFILIATION WITH UCSF

Brown & Toland medical group (B&T) has chosen to end its affiliation with the UCSF medical group effective December 31, 2009. The UCSF medical group will affiliate with Hill Physicians of San Francisco beginning January 1, 2010.  

This change in medical groups could affect Health Net and Anthem Blue Cross PLUS members who use UCSF providers who are currently part of the Brown & Toland medical group.

You should evaluate your medical needs as you may need to take action to change your medical group.  Changes to your medical group cannot be made through UC’s Open Enrollment website.  Instead, follow the instructions below for your medical insurance plan.
  
Health Net HMO/Health Net Seniority Plus 
If you have a UCSF PCP with Brown & Toland, you will be transferred automatically to Hill Physicians of San Francisco effective January 1, 2010, and you will keep your same UCSF PCP. If you would prefer to stay with Brown & Toland and not follow your current UCSF PCP, you will need to contact Health Net Customer Service (1-800-539-4072) and select a new PCP.

If you have a non-UCSF PCP, you will remain with Brown & Toland unless you call Health Net (1-800-539-4072) to switch to Hill Physicians. In mid-October

Health Net will send a letter to all members currently assigned to a UCSF PCP with Brown & Toland informing them of the automatic transfer. Members will be provided with information on what to do if they wish to make another selection. Transition of Care information will also be included in this mailing.

Anthem Blue Cross PLUS (Medicare and non-Medicare)
You will not automatically transfer to Hill Physicians. If you want to keep your UCSF PCP, you must call Anthem Blue Cross (1-888-209-7975) beginning October 1, 2009, to request a change to Hill Physicians of San Francisco medical group effective January 1, 2010.  If your PCP has a closed practice, you should indicate you are a current patient of that provider.  If you are using a UCSF specialist, you will receive a transition of care letter from Anthem Blue Cross PLUS. If you plan to stay with Brown & Toland, you will need to call Anthem Blue Cross to get an authorization to complete your treatment with your current UCSF specialist.     

To ensure changes are effective on January 1, 2010, you should make any needed changes with your plan by December 12, 2009.
