The United Behavioral Health (UBH) transition-of-care benefit ends on June 30, 2008.

The purpose of this benefit has been to allow members the opportunity to complete their course of treatment or provide the time necessary to identify an appropriate treatment provider participating within the UBH network. If continued treatment is necessary, contact the UBH intake counselor (1-888-440-8225) for help to identify an appropriate network provider. The Employee Assistance Program at your location may also be helpful in identifying an appropriate behavioral health care provider.

Note that if a member receiving transition benefits continues to see a non-UBH provider after June 30, 2008, the full cost for the provider’s services must be paid by the member.
