Last month, an addendum to PacifiCare's 1999 Combined Evidence of Coverage (EOC) and Disclosure Form was included in their magazine, HealthBeat. PacifiCare mailed the magazine to all members the week of February 8.

The provisions in the Addendum are effective 1/1/99. None of the changes affect copayments. Some of the changes are enhancements, while others clarify or state items that are already covered under the plan.
