[January 2, 2002]

If you changed medical plans during Open Enrollment, you have probably already received a medical plan ID card from your new plan. (Exception: ID cards for the BluePremier plans have been delayed and plan members should receive them the second or third week of January.) You may begin using the card immediately, since your new coverage is effective January 1, 2002. Be sure to verify that your January 1 paycheck stub (or annuitant benefit check stub) or Surepay statement correctly reflects any change in deductions.

If you received a new ID card from your former plan, please discard it. (Some carriers sent out the cards early, based on their year-end membership.)

Certificates of Creditable Coverage

Your former medical plan will send you a HIPAA Certificate of Creditable Coverage. You should retain this certificate for your records. While you don't need to use this now, it may be useful in the future—for example, if you leave UC employment and want to obtain other coverage.

Plan Booklets

You should receive your new medical plan booklet (Evidence of Coverage, or EOC) and other materials by the end of January 2002.

For More Information

If you have questions about your plan, your coverage, or the materials mentioned above, please call your new plan. Telephone numbers for all UC-sponsored plans are available at www.ucop.edu/bencom/info/hw.html.
