Some employees and annuitants enrolled in the UC Care medical plan have had problems recently obtaining prescriptions. The problem can occur if you fill a prescription before your pharmacy has loaded your personal information into its computer system. To avoid this, be sure to tell the pharmacist that your prescription drug carrier changed from Merck-Medco to Prudential HealthCare (PHC) beginning 1/1/2000, and show them your UC Care I.D. card.

If you encounter this situation and end up paying the out-of-pocket cost for your prescription at a participating pharmacy, there are two ways you can be reimbursed.

If you obtain your prescription at a non-participating retail pharmacy, submit the three items listed above, along with a medical claim form (available from Prudential HealthCare at 1-800-313-3804), to:

Note: When you obtain a prescription at non-participating pharmacy, you must always pay the full cost of the prescription at the time of purchase and then submit a medical claim form to Prudential HealthCare for reimbursement. You will be reimbursed at 80% after an annual prescription deductible ($50 for an individual or $150 for family).

Reimbursement checks are processed twice a month, on the 1st and 15th.

Note: Prudential HealthCare will be mailing a question and answer guide about their prescription drug benefits to UC Care members in late April. We will post the guide on this website when it is available.
