Tax Verification Requests are being mailed this week to 3,733 University of California employees and annuitants, asking them to document tax dependency for approximately 4,500 family members.

Employees and annuitants to receive the mailing are those whose UC-sponsored medical, dental, vision, or legal coverage in 1999 included at least one of the following: stepchild, legal ward, disabled child over age 22, other child enrolled before September 1, 1994, adult dependent relative, child or grandchild of a same sex domestic partner, or a qualified domestic partner who is claimed as the employees or annuitant's tax dependent.

The response deadline for all who receive a Tax Verification Request is Friday, April 28, 2000. Those unable to submit documentation by this date—for example, if they have applied for an extension from the IRS—must respond by the April 28, 2000 deadline, alerting Insurance Services that they need additional time.

Those who do not respond will be notified that the family members are deemed ineligible and will be de-enrolled retroactive to January 1, 1999 or the family member's coverage date, whichever is later. Insurance carriers will also be advised.

Those who recently participated in the Family Member Verification should note that this verification is not the same. The Tax Verification Request occurs annually and involves only tax records for the previous year.
