If you become ill or are injured and will be unable to work for an extended period of time, you should contact your supervisor or department chair and your local Benefits Office. You must be under a doctor's care to apply for disability benefits.

You should apply for benefits from the Short-term and Supplemental Disability Plans as soon as it appears you will be unable to work due to a disabling injury, illness, or pregnancy. To avoid a lapse in your pay, apply for disability benefits while you are still receiving sick leave. Keep in mind that it takes time for the insurance company to compile all the necessary medical and other information to pay a claim.

Each UC-sponsored disability program has its own application form and procedure. Your local Benefits Office and/or Workers' Compensation Office will help you apply for disability benefits. Your supervisor will counsel you about your various options for a leave of absence.
