It's not too late to complete a health assessment for 2008.
This year, employees represented by UC unions, except those in UC-AFT and SETC, are not eligible for StayWell because participation was not agreed to on behalf of their members during the collective bargaining process. In 2009, the incentive for completing StayWell's health assessment will be a $100 gift card for employees and $50 for spouses/domestic partners. Health assessments must be completed by April 15 to qualify for the gift card. UC Kaiser Permanente members and their family members aged 18 and older may also take a health assessment, via its HealthWorks program. Those who take the health assessment are eligible for quarterly drawings for prizes, including $500 gift certificates and iPods. Additional information is available at Kaiser's HealthWorks website.
