As previously announced in this newsletter, the paper group insurance beneficiary designation forms filed at the UC campuses and locations prior to 2004 will be valid only through December 31, 2006. The beneficiary designations at the UC campuses and locations will then be obsolete. To review, name, and change UCRP retirement plan beneficiaries and your group insurance plan beneficiaries directly with UC HR/Benefits, you should visit UC’s At Your Service website.

At the website select "Your Benefits Online," log in and select the link "View Your Beneficiaries" under either "Your Money" or "Your Health and Welfare."

Insurance benefits for employees who do not make online designations or submit a paper form (UBEN 116, available on At Your Service or from your local Benefits Office) for naming beneficiaries will be paid according to the order of succession (spouse or domestic partner, child or children, parents, siblings, estate).
