New Demographic Data Form Now Available New Demographic Data Form Now Available

As a result of new U.S. Department of Education standards, the University of California is required to change the manner in which it collects and reports employee race and ethnicity data to the Integrated Postsecondary Education Data System (IPEDS).
In order to comply with the new requirements, the Demographic Data Transmittal form U5605 and the Payroll Personnel System have been modified.
Employees hired beginning in August 2010 should complete the revised form U5605 in order for the University to meet the new requirements and maintain the current reporting demands for campus affirmative action programs.
