To help protect you and your family from the unforeseen financial hardship of a serious accident that causes death or dismemberment, UC offers retirees and their spouse or same-sex domestic partner Accidental Death and Dismemberment (AD&D) coverage through the UC-sponsored Voluntary Group Accident Insurance Program.

Plan Benefits
The plan provides benefits if the accident results in:

  • accidental death or dismemberment or loss of sight
  • paraplegia, quadraplegia, or hemiplegia
  • coma lasting more than 30 days

Enrollment
You may enroll at any time. Use the form available from Chartis.

Plan Costs
See the chart for current monthly premiums.

To File a Claim
You or your beneficiaries should send a written notice of claim to Chartis within 20 days of your loss. If you require assistance filling out the claim form, call Chartis at 1-800-551-0824.

Naming/Changing Beneficiaries
To name or change your designated beneficiary, call the insurance company to receive a change of beneficiary form in the mail. Once the insurance company accepts a new form, all previous designations are revoked.
